Requirements for the Pre-Diabetic Wellness Program

To enroll in the Diabetes Prevention Program you must be 18 years of age or older, have BMI of ≥ 25 kg/m2 (≥ 23 kg/m2, if Asian American) and must also have no previous diagnosis of type 1 or type 2 diabetes.

In addition, you must take the Pre-Diabetes Screen Test CDC/ADA Pre-diabetes Screening Test score of 5 or above and submit your score.
